Fibromyalgia Treatment: Historical perspectives on the development of the fibromyalgia

Chronic Pain is the most prominent symptom of fibromyalgia.
Fibromyalgia patients have an increased sensitivity to many different sensory stimuli.The pain of fibromyalgia can be exacerbated by sensory stressors of noise, weather change, anxiety, or emotional stress.
Studies reveal that this is due to the fact that FM patients experience a lack of level 4 or deep rejuvenating sleep. Although they can fall asleep quickly, their deep sleep is interrupted by burst of awake-like brain activity. A vast majority of FM patients experience non-restorative sleep. Lack of rejuvenating sleep leaves the FM patient highly fatigued. Fatigue occurs in 90% of fibromyalgia patients.
Consequently, patients often awaken in the morning without feeling fully rested. Some patients awaken with muscle aches or a sensation of muscle fatigue as if they had been ‘working out’ all night!
Depression, Axiety & StressMore than half of fibromyalgia patients experience additional symptoms such as; poor concentration, forgetfulness, mood changes, irritability, depression, increased stress and anxiety.
FibroFogIs a commonly reported symptom of Fibromyalgia. Fibromyalgia patients often describe multiple sensations of fatigue and listlessness combined with transitory states of confusion, poor attention and concentration, and short-term memory loss.
Additional symptoms of fibromyalgia include headaches, dizziness, numbness/tingling, and abdominal pain. Over 80% of fibromyalgia patients sufferchronic headaches.
What Causes Fibromyalgia?
Although there is no definitive agreement on one single cause for fibromyalgia, current research has uncovered clues to what “triggers” or “predisposes” an individual to develop fibromyalgia.
  1. Stress and/or depression
  2. Traumatic emotional experience (divorce, abuse, etc.)
  3. Traumatic physical experience (car accident, neck injury*, etc.)
  4. Systemic toxicity (food, air, water)
  5. Chronic fatigue syndrome
  6. Sleep disturbances
  7. Family history
  8. Localized or systemic infection
  9. Nutrient deficiencies due to poor nutritional habits
  10. Lack of exercise and cardiovascular activities
GITract/Digestion and Fibromyalgia
Studies reveal that approximately 70% of Fibromyalgia sufferers also suffer from chronic digestive problems.Complications/Contributors:

Cortisol
Research has revealed that chronic stress and fatigue, both experienced by and with FM patients, affect hormone levels. Chronically high stress levels lead to a high level of the hormone cortisol. High cortisol is associated with depression and obesity, both which are seen in high percentages with Fibromyalgia.
Estrogen
Estrogen has in influence on serotonin levels, and in the presence of lower estrogen levels, serotonin levels are also lowered. This may help to explain why many Fibromyalgia patients experience worsened symptoms during their periods, and at menopause.
Thyroid Gland
Estrogen also affects the thyroid gland and thyrotropin-releasing hormone (TRH), the hormone that regulates the thyroid gland by causing the release of thyroid stimulating hormone (TSH). Lower levels of TSH affect the thyroid gland are important in metabolism, and are thought to play a role in the fatigue factor seen in Fibromyalgia patients.
DHEA
DHEA is an adrenal gland hormone that has been linked to depression and fatigue, and has been found in lower levels in Fibromyalgia patients.
Growth Hormone
Growth hormone is almost exclusively produced during deep sleep (level 4), however it is also produced during exercise – both of which are decreased in Fibromyalgia patients.Growth hormones are produced in the anterior pituitary gland are important in metabolism, particularly fat burning and muscle building.
Fibromyalgia and the Immune System
The relationship between chronic stress and the function of the immune system has been well established. Recent research has linked fibromyalgia and a depressed immune system:
  • Natural killer cells that fight bacteria are less active in Fibromyalgia patients. This may help to explain why Fibromyalgia patients are prone to infections.
  • Depression has also been linked to a depressed immune system
  • Approximately 60% of Fibromyalgia patients also have bacterial and virus infections that have been linked to a depressed immune system
  • FM patients are more likely to have gastrointestinal tract viral antibodies
  • Energy Production and Fibromyalgia
  • Recent research has revealed that Fibromyalgia patients are deficient in certain compounds that are required for the body to produce energy.
The 3 main compounds lacking are:
  1. Magnesium
  2. Substrate oxygen
  3. Phosphates
Deficiencies of these key compounds seriously impede the energy producing Kreb’s Cycle decreasing the body’s ability to use oxygen for muscle energy. These deficiencies clearly lead to fatigue, depression and muscle pain – which are major symptoms found in Fibromyalgia patients.
Magnesium and Malic Acid – Detoxifiers
Aluminum’s role as a detrimental toxin has been clearly established in the literature. Many symptoms experienced by FM are the same as those found with aluminum toxicity. Therefore detoxifying the body of this deadly metal is critical.
It has been found that by combining magnesium and malic acid can effectively decrease the levels of aluminum in all vital organs, especially the brain. A research study on 15 Fibromyalgia patients found that after taking 1,200-2,400 mg of malic acid with 30-600 mg of magnesium daily that all 15 patients reported significant pain relief within 48 hours of treatment!
Manganese and Fibromyalgia
Manganese has been found to directly influence metabolic rate (via its involvement in the production of hormones that directly impact the thyroid gland).
Diagnosing Fibromyalgia

Step 1: Rule out other illnesses
The following illnesses must be ruled out first (not a complete list):
  1. Candidadiasis (yeast infection)
  2. Chronic fatigue syndrome
  3. Chronic infection
  4. Depression or related psychological conditions
  5. Digestive disorders, including IBS
  6. Hypothyroidism
  7. Immune deficiencies
  8. Lyme disease
  9. Myofascial pain syndrome

Step 2: Fibromyalgia Diagnosis Criteria
1. Widespread history of pain.The American College of Rheumatology (ACR) developed and published the tender point classification for fibromyalgia diagnosis. The patient must have tender points in 11 of the 18 locations described in the ACR criteria. Pain is produced by placing the equivalent of 4 pounds of pressure on the tender point locations. This amount of pressure should not normally produce pain.
2. The pain must be chronic (longer than 3 months).
3. The pain must be located bilaterally (meaning found on both the right and left sides of the body) and also in the upper and lower parts of the body. This is defined as pain found in all 4 quadrants, upper right, upper left, lower right, lower left.

he ACR landmark study published in the February 1990 issue of Arthritis and Rheumatism, was proof positive that fibromyalgia was indeed a syndrome and diagnosis.
1. The study consisted of 558 patients, 265 of which were classified with fibromyalgia syndrome.2. The remaining 293 were not normal healthy patients but instead had many other painful conditions, such as, back pain, neck pain, trauma-related pain syndromes, rheumatoid arthritis, lupus, and many others.3. When the examiners were asked to pick the fibromyalgia patients, using the ACR 11/18 criteria, they were able to pin point with 88% accuracy those that were classified with fibromyalgia syndrome!

Once the diagnosis of fibromyalgia is made, it is important to create your fibromyalgia team. This is your personal team that is going to fight the battle with you, and help you to live your best life!
Fact or Fiction?
Fibromyalgia cannot be cured.Fibromyalgia CAN be cured!Fibromyalgia cannot be cured is what most fibromyalgia patients are told. They have to live with it for the rest of their lives. This is absolutely not true. However, it will not be easy, and there is no magic pill. It takes fierce determination and dedication to yourself, if you are going to live your best life now!

  1. I am not anti–all medications. Some medications are life saving. I am opposed to the mindset that all of society’s problems can be overcome by popping a pill, especially fibromyalgia.

Medication’s Role in Fibromyalgia
Dr. Joseph M. Elrod goes on to say,“Overall, this treatment method (medication) has not been very effective and, as a result, the treatment of fibromyalgia has been frustrating for both patients and physicians.The major problem with drug treatments is that often, instead of getting to the source of the problem, they merely mask the symptoms.
Also, there are almost always side effects, and very often these side effects can exacerbate the original problem and/or cause other problems that are as serious as the original condition.”
You Deserve Another Approach
In a review in the 2009 Journal of Rheumatology, fibromyalgia researcher H.A. Smythe writes (regarding medication for FM), “patients receive some benefit, but when side effects make the patient dull, lethargic, or overweight, neither their goals nor those of society are satisfactorily met”.
Dull – Lethargic – Overweight: Medication is NOT the Solution.
Is it worth it?The chances that a medication will effectively reduce your fibromyalgia symptoms are usually less than 50 percent, and quite often the drug will produce intolerable side effects. Given these odds, the trial and error process is a difficult road for fibromaylgia patients to take.

Fibromyalgia medication: PROVIGILThe common side effects consisted of headache, nausea, nervousness, anxiety, dizziness, and insomnia. One-half experienced intolerable side effects.

RESEARCH: Fibromyalgia and Exercise
This study evaluated combining aerobic exercises with strength training exercisesAerobic exercise walking on treadmill, initially for 20 min and increasing up to 30 min as the patient tolerated.
  1. Aerobic exercise intensity was 60–70% of age-adjusted maximum heart rates (220 minus age in years).
  2. Patients in the strengthening exercise group consisted of upper and lower limb muscles and trunk muscles, initially with 4–5 repetitions and progressing to 12 repetitions gradually. Patients began with resistance levels they could do easily, and weight was increased gradually according to patient’s tolerance.
  3. Pain, sleep, fatigue, tender point counts, 6-min walk distances, depression, and quality of life all improved
Note: No patient experienced musculoskeletal injury or exacerbation of fibromyalgia-related symptoms during the intervention.

Sixty patients undertook 30 treatments combining ischemic compression and spinal manipulation. At the midway point, participants reported a significant lessening of pain and an improvement in quality of sleep and fatigue level.
After 30 treatments, respondents reported the following:

  2. 63.5% Improvement in sleep quality
  3. 74.8% Improvement of in fatigue level
In 2007 the American College of Physicians and the American Pain Society included spinal manipulation as a viable treatment option.

Healthy Snack Recipes


Snack Solutions: Primal Alternatives For Non-Primal Snacks

Welcome! If you want to lose weight, gain muscle, increase energy levels, reduce stress or just generally look and feel healthier you've come to the right place.
In one sense Grok had it easy. He never had potato chips or candy bars in his diet. You, on the other hand, likely used to eat these foods in some way, shape or form before you found the Primal lifestyle. And, while you’re happy with all the changes that Primal Living has afforded, every now and then, when you least expect it, a craving for a cheese doodle – of all things! – sneaks up.
Well, fear no more, because we’ve figured out some great Primal alternatives to some of your favorite not-so-Primal foods. Granted, they’re not perfectly Primal and may contain a bit more sugar than we usually recommend, but they’ll beat store-bought, HFCS-laden, processed junk food any day.

Primal Veggie Chips:

You’ve seen veggie chips on store shelves, but if you check the label you’ll usually find potato as the first ingredient in the list. Skip the cleverly marketed potato chips and try this recipe that tastes MUCH better than those stale crisps:
Ingredients: 
1 large, ripe eggplant
2 eggs
2 cups cheese
Method:
Cut ends from eggplant and shred in a food processor. Mix shredded eggplant with egg and cheese. Lump mixture onto a pre-greased cookie shape and smooth out to make 6 8″ circles. Bake at 450 degrees F in a pre-heated oven for 12 minutes. Loosen and flip the circles. Bake on other side for a further five minutes. Once cooked, remove from oven and cut rounds into triangles with a pizza cutter. Let cool on a rack for 6-8 hours. Once dried, sprinkle with popcorn seasoning, Old Bay, garlic salt or any other seasoning of your choice. Eat immediately or store for up to one week in an airtight Ziploc bag.

Primal Trail Mix

Store-bought Trail Mix is typically loaded with a ton of sugar – this version, however, weeds out the bad stuff and leaves you with all the satisfying, delicious stuff!
Ingredients:
1 cup raw or roasted almonds
1 cup pumpkin seeds
½ cup sunflower seeds
1 cup blueberries
½ cup raisins
Method:
Combine all ingredients in an airtight container and store in a cool, dry place (it really can’t get any easier!)

Better-Than-Store-Bought Apple Chips

You’ve got the savory down, now it’s time for a little something sweet…
Ingredients:
2 cups unsweetened apple juice (if possible, juice your own).
1 cinnamon stick
2 large apples
Method:
In a large pot, combine apple juice and cinnamon stick and bring to a low boil. Meanwhile, remove top and bottom of apple and slice crosswise to make 1/8 thick “chips.” With a slotted spatula, place apples into boiling juice and cook 4-5 minutes or until apples become near translucent. Use the spatula to remove apple slices from juice and place on a clean kitchen towel and pat dry. Arrange slices on a cake cooling rack placed on a cookie sheet (to catch drips!) and place racks on middle shelf in 250 degree oven. Bake 30-40 minutes until apple slices turn golden brown and are almost dry to the touch. Let cool and serve either as is, or with a light dusting of cinnamon.

Almost-Chocolate Carob Treat

Ingredients:
1 cup toasted unsweetened carob
1 ½ cups pecan meal
½ cup or less coconut oil
Method:
In a large bowl, mix all ingredients together. Spread in a baking tin lined with parchment paper. Will make a slightly softer version of fudge, but the recipe’s creator swears this carob treat helped her kick what she calls a “significant” chocolate habit. Not that we have anything against chocolate!

Blueberry “Candies”

When a sugar craving hits, this blueberry candy ought to fit the bill:
Ingredients:
5 cups blueberries
4 tsp cinnamon
1 ½ tsp ginger
2 egg whites
1/4 cup raw honey (yes, we know that honey is pretty much pure sugar, but when it’s spread out across 5 cups, it won’t have too much of a glycemic impact)
1 tsp vanilla extract
In a large bowl, whisk egg whites until frothy. Add in honey and vanilla and stir until combined. With a slotted spoon, add blueberries to the egg mixture. Remove and roll in a small bowl filled with a mixture of cinnamon and ginger. Repeat until all blueberries are covered. Using the same slotted spoon, transfer the coated blueberries onto a plastic dehydrator tray. Dehydrate for 24 hours or until dry. After 8-12 hours, or when you see that one side is dry enough, turn them over to dry other side. Serve or store in an airtight container.

All-Natural Fruit Roll-Ups

Who says fruit roll-ups are just for kids? This stow and go snack is great for throwing in your purse or backpack for a quick pick-me-up!
Ingredients:
2 large apples
2 cups strawberries
1 tsp cinnamon
¼ cup purified water
Clean, core and dice apples. Add diced apples and strawberries in a blender and add a ¼ cup of purified water and cinnamon and process about 30 seconds or until smooth. Pour mixture on a teflex sheet (a Teflon-coated sheet commonly used to dehydrate delicate foods) and place in a plastic dehydrator. Dehydrate for 6-8 hours, remove teflex and flip fruit. Continue drying another 4-6 hours or until desired consistency is achieved. Use a pizza cutter to slice into snack-size pieces.
The last two recipes may be done in an oven if you don’t have a food dehydrator. Times will vary so keep an eye on it, but they will likely take about as long. A good rule of thumb when using an oven to dehydrate is to set the temperature between 100 and 150 degrees Fahrenheit, and keep the oven slightly cracked for the duration of the dehydration.

Crave Foods Could be your Leptin levels how to fix it Cravings? Fix Your Leptin!
Ever had trouble sticking to a diet?
Crave foods?
Overweight?
Want to snack, especially at night?
These are all indications that you could have some Leptin issues. Leptin is a master hormone in the body that controls hunger and feelings of satiety.Leptin is secreted by adipose (fat) tissue, so the more overweight a person is, typically, the higher his leptin levels.
According to Mark’s Daily Apple: “Leptin is the lookout hormone – the gatekeeper of fat metabolism, monitoring how much energy an organism takes in. It surveys and maintains the energy balance in the body, and it regulates hunger via three pathways:
  • By counteracting the effects of neuropeptide Y, a potent feeding stimulant secreted by the hypothalamus and certain gut cells.
  • By counteracting the effects of anandamide, another feeding stimulant.
  • By promoting the production of a-MSH, an appetite suppressant.”
It is also directly tied to insulin levels. Many people these days are Leptin resistant and there are many health problems tied to this problem. High leptin levels have been tied to high blood pressure, obesity, heart disease and stroke, as well as blood sugar related problems.
High levels of Leptin and the accompanying leptin resistance can also decrease fertility, age you more quickly and contribute to obesity. If you’re trying to lose weight or improve a health problem, chances are you have Leptin resistance. If you can’t seem to stick to health changes, chances are you have Leptin resistance.
In other words, if you want to make lasting health changes or lose weight and keep it off, you have to fix your leptin. The good news is: if you’ve failed at diets or health changes in the past, it was likely because you failed to regulate your Leptin levels and doing so can help you finally make lasting changes. Leptin resistance and its related problems are a complex problem involving the endocrine system and reversing them requires more than simple calorie restriction or will power.

The Downward Spiral

Leptin is the satiety hormone produced by fat cells, so it would seem logical that those with more fat cells would produce more Leptin, which would signal the body to eat less food and weight would normalize. This over-simplified idea is great in theory, but doesn’t always occur, especially in those with damaged metabolisms or endocrine problems (which includes most overweight people).
According to this article: “The problem is not in the production of leptin, but rather, studies show that the majority of overweight individuals who are having difficulty losing weight have a leptin resistance, where the leptin is unable to produce its normal effects to stimulate weight loss. This leptin resistance is sensed as starvation, so multiple mechanisms are activated to increase fat stores, rather than burn excess fat stores. Leptin resistance also stimulates the formation of reverse T3, which blocks the effects of thyroid hormone on metabolism (discussed below).”
So, the person is eating excess food but the body thinks it is starving and tells the person to eat more. It’s easy to see how this cycle could contribute to weight gain!

Factors that Contribute to Leptin Resistance

As with all hormone issues, Leptin resistance is a complex issue with no singular cause, but there are many factors that can negatively impact Leptin levels including:
    • Fructose consumption (especially in forms like High Fructose Corn Syrup)
    • High stress levels
    • Consumption of a lot of simple carbs
    • Lack of sleep
    • High insulin levels (vicious cycle here)
    • Overeating
    • Exercising too much, especially if your hormones are already damaged
    • Grain and lectin consumption

    How to Fix Leptin Resistance:

    As I said, this is a complex problem, but not an irreversible one. Dr. Jack Kruse (a neurosurgeon) and Stephan Guyenet (an obesity researcher) have both written in depth about the causes of Leptin imbalance and ways to reverse it. I highly recommend these articles for more information about their approaches: Changing the Body’s Setpoint, Factors that affect Leptin, and Dr. Kruse’s Leptin Prescription.   The book Mastering Leptin also has a much more in depth explanation and suggestions.
    In short, the (non-negotiable) factors that will help improve leptin response are:
    • Eating little to no simple starches, refined foods, sugars and fructose
    • Consuming a large amount of protein and healthy fats first thing in the morning, as soon after waking as possible. This promotes satiety and gives the body the building blocks to make hormones. 
    • Be in bed by ten (no excuses) and optimize your sleep!
    • Get outside during the day, preferably barefoot on the ground, in mid-day sun with some skin exposed. There are many reasons this is helpful and I’ll be explaining them soon)
    • DON’T SNACK!!! When you are constantly eating, even small amounts, during the day it keeps your liver working and doesn’t give hormones a break. Try to space meals at least 4 hours apart and don’t eat for at least 4 hours before bed. This includes drinks with calories but herbal teas, water, coffee or tea without cream or sugar are fine.
    • Don’t workout at first. If you are really Leptin resistant, this will just be an additional stress on the body. Let your body heal a little first, then add in the exercise.
    • When you do exercise, do only sprints and weight lifting. Walk or swim if you want to but don’t do cardio just for the sake of cardio. It’s just a stress on the body. High intensity and weight lifting, on the other hand, give the hormone benefits of working out without the stress from excess cardio and are great after the first few weeks. Also, workout in the evening, not the morning, to support hormone levels.
    • Remove toxins from your life as these are a stress on your body. There will be more specifics on how to accomplish this in the next few weeks, but getting rid of processed foods, commercial deodorants (make your own) and comercial soap (use microfiber) will go a long way!
    • Eat (or take) more Omega-3s (fish, grassfed meats, chia seeds) and minimize your Omega-6 consumption (vegetable oils, conventional meats, grains, etc) to get lower inflammation and help support healthy leptin levels.

    What Are Fermented Foods?

    Fermented foods are foods that have been through a process of lactofermentation in which natural bacteria feed on the sugar and starch in the food creating lactic acid. This process preserves the food, and creates beneficial enzymes, b-vitamins, Omega-3 fatty acids and various strains of probiotics.
    Natural fermentation of foods has also been shown to preserve nutrients in food and break the food down to a more digestible form. This, along with the bevy of probiotics created during the fermentation process could explain the link between consumption of fermented foods and improved digestion.
    Cultures around the world have been eating fermented foods for years, from Sauerkraut in Germany to Kimichi in Korea and everywhere in between. Studies have even shown the link between probiotic rich foods and overall health (PDF).Sadly, with the advances in technology and food preparation, these time-honored traditional foods have been largely lost in our society.

    Where Have All the Fermented Foods Gone?

    The amount of probiotics and enzymes available in the average diet has declined sharply over the last few decades as pasteurized milk has replaced raw, pasteurized yogurt has replaced homemade, vinegar based picked and sauerkraut have replaced traditional lacto-fermented versions…the list goes on.
    Even the much dreaded grains were safer to eat in earlier times since their preparation included soaking, sprouting and fermenting, which largely reduces the anti-nutrient content and makes them less harmful (I still didn’t say good!)
    Instead of the nutrient rich foods full of enzymes and probiotics that our grandparents probably ate, the average diet today consists mainly of sugar laden, lab created dead foods.

    Why Eat Fermented Foods?

    Besides the fact that they taste great and really grow on you, there are several great reasons to start making and eating fermented foods:
    1. Probiotics- Eating fermented foods and drinking fermented drinks like Kefir and Kombucha will introduce beneficial bacteria into your digestive system and help the balance of bacteria in your digestive system. Probiotics have also been shown to help slow or reverse some diseases, improve bowel health, aid digestion and improve immunity!
    2. Absorb Food Better- Having the proper balance of gut bacteria and enough digestive enzymes helps you absorb more of the nutrients in the foods you eat. Pair this with your healthy real food diet, and you will absorb many more nutrients from the foods you eat. You won’t need as many supplements and vitamins, and you’ll be absorbing more of the live nutrients in your foods.
    3. Budget Friendly- Incorporating healthy foods into your diet can get expensive, but not so with fermented foods. You can make your own whey at home for a couple of dollars, and using that and sea salt, ferment many foods very inexpensively. Drinks like Water Kefir and Kombucha can be made at home also and cost only pennies per serving. Adding these things to your diet can also cut down on the number of supplements you need, helping the budget further.
    4. Preserves Food Easily- Homemade salsa only lasts a few days in the fridge- Fermented homemade salsa lasts months! The same goes for sauerkraut, pickles, beets and other garden foods. Lacto-fermentation allows you to store these foods for longer periods of time without losing the nutrients like you would with traditional canning.

    Bring on the Bacteria! How to Incorporate Fermented Foods Into Your Diet

    I’ll be delving into this more in the next few weeks and providing some recipes, but adding fermented foods to your diet can be an easy process (and can save you money on probiotics and digestive enzyme supplements!)
    On a basic level, you can make foods like sauerkraut with just cabbage, water and salt on your counter (that recipe can be adjusted down to make 1 head of cabbage worth in a quart size jar).
    You can also incorporate fermented drinks like Water Kefir and Kombucha (cultures available here) which are inexpensive to make and can be carbonated like soda!
